Job summary

Carle Health is seeking a licensed Physical Therapist to evaluate and treat patients of all ages in New York, guiding a comprehensive plan of care and adjusting it based on patient response and evidence. Qualifications

• Bachelor's degree is required.
• Licensed Physical Therapist (PT) in IL per IDFPR.
• Basic Life Support (BLS) within 30 days - American Heart Association.
• Attend and satisfactorily complete all required continuing education regarding the care of acute stroke patients.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed from bed, chair and floor, if applicable.
• Demonstrates respect by treating patients and staff with dignity and empathy.
• Designs an appropriate plan of care that integrates goals, treatment, outcomes, and discharge plan.
• Educates patients, family, caregivers, students, and other health care providers.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examination, selecting reliable methods relevant to chief complaint and history.
• Produces documentation in accordance with CTSCP67.
• Screens patients to determine effectiveness and need for therapy services.
• Monitors care delivered by support personnel and provides feedback.
• Completes annual education for DNV Hip and Knee Replacement Program requirements if applicable.
• Collaborates with patient and family and provides education/training as necessary.
• Provides supervision to PTA and rehabilitation technicians.
• Communicates patient performance and discharge needs with the medical team.
