Job summary

Sign-on bonus and relocation assistance are available for moves over 50 miles. New grads may qualify for a $5,000 sign-on bonus; employees with 1+ year experience may receive $7,500.

Qualifications

• Must have a Bachelor's Degree in Physical Therapy.
• Licensed Physical Therapist (PT) in Illinois.
• BLS certification within 30 days - American Heart Association.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance as needed and uses correct body mechanics to minimize risk.
• Treats patients with dignity and empathy, engaging those needing services.
• Designs and adjusts a plan of care based on goals, treatment, outcomes, and discharge planning.
• Educates patients, family and staff using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes.
• Performs examinations and selects appropriate assessment methods based on history and screening results.
• Produces documentation per CTSCP67 policy.
• Screens patients to determine need for continued therapy and uses support personnel appropriately.
• Completes annual education to comply with DNV Hip and Knee Replacement Program (if applicable).
• Evaluates impairments and establishes plans of care for orthopedic and neurologic conditions.
• Documents daily progress in therapy EMR and supports modality use.
