Job summary

Cornerstone Rehab is seeking a PRN Physical Therapy Assistant to deliver care to adult and geriatric patients across its partner centers in Kentucky. You will collaborate with a diverse team of clinicians to implement skilled rehab programs.

The role requires valid PTA licensure in the state and experience in SNF or acute care settings; travel to nearby centers is expected, with support for professional development.

Qualifications

• Must have Valid State PTA License and credentials.
• SNF and/or Acute care experience preferred.

Responsibilities

• Provide physical therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care in accordance with the patients' needs and capabilities.
• Travel to nearby centers as needed.
