Qualifications

• Valid State PTA License (Kentucky).
• SNF and/or Acute care experience.

• Provide physical therapy to adult and geriatric patients.
• Prepare documentation, progress notes, and discharge summaries.
• Establish a plan of care tailored to patient needs and capabilities.
• Travel to nearby centers as needed.
