Santa Cruz Post Acute is seeking a compassionate and motivated Physical Therapy Assistant (PTA) to join our exceptional rehabilitation team. Our skilled nursing facility is dedicated to delivering high-quality, resident-centered care in a supportive and collaborative environment. We offer full-time, part-time, and PRN opportunities with flexible scheduling.
The Physical Therapy Assistant works under the supervision of a licensed Physical Therapist to provide therapeutic treatments that enhance residents’ mobility, strength, balance, endurance, and overall functional independence.
Deliver direct physical therapy treatments following individualized plans of care developed by the Physical Therapist
Assist residents with therapeutic exercises, gait training, mobility programs, and functional activities
Accurately document daily treatment sessions, resident progress, and responses to therapy in compliance with facility and regulatory standards
Collaborate with the Physical Therapist, therapy team, nursing staff, and interdisciplinary team members to support resident-centered goals
Educate residents and caregivers on exercise programs, safety strategies, and mobility techniques
Maintain compliance with state and federal regulations and professional standards of practice
Current Physical Therapy Assistant (PTA) license in the state of California
Graduate of an accredited PTA program
Excellent communication, teamwork, and documentation skills
SNF experience preferred, but new graduates are encouraged to apply
Starting at $45 per hour (DOE)
