Cypress Pointe Health and Wellness is seeking a dedicated Physical Therapist Assistant (PTA) for a full-time position in a Skilled Nursing setting in Houston, TX. The PTA will support rehabilitation, help implement care plans, and coordinate care with the care team to ensure optimal patient outcomes.

Qualifications

• Texas PTA license is required.
• Experience in Skilled Nursing or long-term care preferred.
• Experience with EMR software.

Responsibilities

• Assist rehabilitation of patients with physical or mental disabilities.
• Help with implementation of physical therapy care plans.
• Coordinate care with the entire care team.

Responsibilities

• Assist with facilitation of rehabilitation of patients with emotional, physical or mental disabilities
• Assist with implementation of physical therapy care plans
• Assist with coordination of care with entire care team
• Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members

Must Haves

• PTA -Physical Therapist Assistantin the State of Texas
• Skilled Nursing, Healthcare (Long Term Care) or Veterans Homeexperience highly preferred
• Experience with EMR
