Job summary

Cypress Pointe Health & Wellness invites a dedicated Physical Therapist Assistant (PTA) to join our team in Houston, Texas. This full-time PTA role supports skilled nursing patients and collaborates with a multi-disciplinary care team to implement PT care plans and optimize patient outcomes.

Qualifications

• PTA credential required in Texas.
• Experience in Skilled Nursing or Veterans Home is preferred.

Responsibilities

Responsibilities

• Assist with facilitation of rehabilitation of patients with emotional, physical or mental disabilities
• Assist with implementation of physical therapy care plans
• Assist with coordination of care with entire care team
• Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members

Must Haves

• PTA -Physical Therapist Assistant in the State of Texas
• Skilled Nursing, Healthcare (Long Term Care) or Veterans Home experience highly preferred
• Experience with EMR
