Job summary

Cypress Pointe Health and Wellness is seeking a Physical Therapist Assistant (PTA) for a full-time position in a skilled nursing setting. The PTA will support rehabilitation, help implement care plans, and coordinate with the care team to ensure quality patient outcomes.

Qualifications

• Licensed PTA (Physical Therapist Assistant) in Texas.
• Experience in Skilled Nursing or long-term care preferred.
• Experience using EMR systems.
• Demonstrates compassion and dedication to patient care.

Responsibilities

• Assist with rehabilitation of patients with disabilities.
• Assist with implementation of physical therapy care plans.
• Assist with coordination of care with the care team.

Responsibilities

• Assist with facilitation of rehabilitation of patients with emotional, physical or mental disabilities
• Assist with implementation of physical therapy care plans
• Assist with coordination of care with entire care team
• Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members

Must Haves

• PTA -Physical Therapist Assistantin the State of Texas
• Skilled Nursing, Healthcare (Long Term Care) or Veterans Homeexperience highly preferred
• Experience with EMR
• Truly cares for people and will go above and beyond to provide outstanding patient care
