Job summary

Autumn Leaves Memory Care in Texas is seeking a dedicated Physical Therapist Assistant to join our in-house rehabilitation team. You will assist with rehabilitation of patients and support care plans under licensed therapists.

Ideal candidates hold TX PTA licensure, experience in skilled nursing or long-term care, and comfort with EMR systems. We offer flexible schedules, competitive pay, PTO and benefits, and a supportive team focused on quality patient care.

Responsibilities

• Assist with facilitation of rehabilitation of patients with emotional, physical or mental disabilities
• Assist with implementation of physical therapy care plans
• Assist with coordination of care with entire care team
• Communicating effectively with patients, caregivers, healthcare providers, office support and other care team members

Must Haves

• PTA -Physical Therapist Assistant in the State of Texas
• Skilled Nursing, Healthcare (Long Term Care) or Veterans Home experience highly preferred
• Experience with EMR
