Job summary

PACS is seeking a Staff Physical Therapist to evaluate and treat residents, maintain detailed medical records, and communicate with families and physicians. You will develop and update treatment plans, supervise PT assistants, and support rehabilitation goals across the facility.

The role emphasizes collaboration with the health team, adherence to safety and infection control, and active participation in quality improvement and staff training initiatives.

Qualifications

• Bachelor’s degree in Physical Therapy required.
• Licensed Physical Therapist in the state; continuing education per state practice.
• CPR certification may be required by licensure.

Responsibilities

• Evaluate residents to determine physical therapy needs and rehabilitation potential.
• Develop and update individualized treatment plans to improve mobility and independence.
• Supervise PT assistants and coordinate with nursing and interdisciplinary teams.
• Communicate progress and discharge planning with physicians, families, and staff.
• Maintain accurate documentation in medical records and adhere to compliance requirements.
• Educate residents and families on home exercise programs and safety strategies.
