Job summary

Fountain Valley Post Acute is seeking a licensed Physical Therapist Assistant to support the Staff Physical Therapist in delivering resident-focused care in a skilled nursing setting. You will assist with treatments, document precisely, maintain a safe environment, and communicate progress to the care team, residents, and families.

This role emphasizes adherence to California PTA standards and quality improvement. Experience with Net Health and Point Click Care is preferred.

Qualifications

• Associate degree from an accredited PTA program.
• Proficient with Net Health and Point Click Care software.

Responsibilities

• Treat residents as directed by Physical Therapist.
• Maintain documentation in accordance with facility and payer requirements.
• Maintain a safe treatment environment per infection control and safety protocols.
• Communicate progress and discharge planning with physicians, nursing staff, and families.
• Provide training to Restorative Aides.
• Educate residents and families on treatment goals and home exercise programs.
• Support care planning and equipment recommendations.
• Confidentiality of residents' and company information.
• Contribute to quality assurance and program development.
