Job summary

Pleasant Hill Post Acute is seeking a compassionate Physical Therapist Assistant (PTA) to join our rehabilitation team on a PRN basis in Pleasant Hill, CA, working under the supervising Physical Therapist.

You will deliver skilled treatments to promote mobility, independence, and safety, with a flexible, as-needed schedule. An Associate Degree and CA PTA license are required; prior SNF or post-acute experience is preferred.

Qualifications

• Associate Degree from an accredited PTA program.
• Current California PTA license.
• Skilled nursing or post-acute rehab experience preferred.

Responsibilities

• Provide skilled physical therapy treatments as directed by the supervising PT.
• Assist residents in achieving mobility and functional goals.
• Document accurately and timely in charting per policy.
• Maintain a safe, clean treatment environment and follow infection control protocols.
• Communicate progress and changes with the supervising PT and care team.
• Collaborate with nursing and other team members as needed.
• Educate residents and families on home exercise programs and safety.
