Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Hadley, MA. The role supports the Physical Therapist in delivering patient-focused therapy services and contributes to treatment planning and resident care outcomes.

Full-time position with a $2,500 sign-on bonus; PRN option also available. Requires an active PTA license in Massachusetts and an associate degree in physical therapy, with ongoing professional development expected.

Qualifications

• Ability to provide PT services under supervision of Physical Therapist.
• Participate in screenings and evaluations as directed.
• Maintain accurate documentation in NetHealth EMR as required.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation and in treatment planning under supervision.
• Assist in establishing resident treatment schedules with the PT and team.
• Document therapy sessions and progress per CMS requirements.
