Evolve Therapy Services is seeking a dedicated Physical Therapy Assistant for an in-house Skilled Nursing Facility in Hadley, MA. You will provide therapy services under the supervision of a Physical Therapist and help develop treatment plans to improve residents’ mobility and function.

Responsibilities include screenings, participation in group and individual treatments, and CMS-compliant documentation in NetHealth EMR.

Qualifications

• Active PTA license required.
• Commitment to ongoing continuing education to maintain licensure.
• Ability to work in a team-based care setting with PTs, other staff, and residents.
• Documentation responsibilities in NetHealth EMR and CMS expectations.

Responsabilités

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention/treatment.
• Assist in evaluation and treatment planning with targets and goals.
• Deliver individual and group therapy per treatment plan (strength, mobility, balance, ROM).
• Maintain CMS-compliant documentation in NetHealth EMR.
