Evolve Therapy Services is seeking a Physical Therapy Assistant to provide in-house skilled nursing facility therapy in Kingsport, TN. The PRN position supports the Physical Therapist with resident care, screenings, treatment plans, and education.

Responsibilities include individual and group therapies, schedule coordination, documentation in NetHealth EMR, and ongoing licensure maintenance. The role emphasizes compassionate, team-based patient care and adherence to CMS standards.

Qualifications

• Must have an active Physical Therapy Assistant license.
• Associate's degree in physical therapy required.
• Ongoing continuing education to maintain licensure.
• PRN position requires flexibility and reliability.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in the evaluation under the Physical Therapist.
• Assist in establishing a target list with short- and long-term goals and treatment plans.
• Conduct individual and group treatment per the treatment plan across mobility, strength, gait, balance, and ROM activities.
• Assist in selecting techniques/media and adapt as indicated by resident performance.
• Monitor residents' response and modify plan in consultation with the Physical Therapist.
• Establish and modify individual resident schedules with the PT and team members.
• Train resident, family and staff in the use of equipment.
• Participate in resident, family, and staff education.
• Maintain compliant documentation in NetHealth EMR per CMS expectations.
