Job summary

Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Webster, MA. The role supports the Physical Therapist and develops treatment plans to enhance resident function and quality of life.

The PTA will conduct screenings, perform group and individual treatments, and participate in ongoing education to maintain licensure, with documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Assume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist in evaluating and developing target goals and treatment plans with the Physical Therapist.
• Provide individual and group treatment as outlined in the treatment plan.
• Assist in selecting techniques/media and adapt as indicated by resident performance.
• Educate residents, families and staff, and document appropriately in EMR.

Education

Associate's degree in physical therapy

Active Physical Therapy Assistant license

Continuing education to maintain licensure
