Evolve Therapy Services, LLC is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Webster, MA. The role supports residents with therapy services and requires active licensure in the relevant state and ongoing CEU participation.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Commitment to ongoing continuing education to maintain licensure.

Responsibilities

• Provide physical therapy services to residents following established standards and procedures.
• Document and bill therapy services in accordance with company procedures.
• Evaluate residents and develop short- and long-term treatment goals.
• Conduct patient screenings and determine need for intervention or treatment.
• Communicate evaluation findings and treatment recommendations to residents and families.
• Complete compliant CMS documentation in NetHealth EMR.

Education

Bachelor of Science Degree in Physical Therapy
