Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Webster, MA. You will work under the supervision of a Physical Therapist to provide resident services.

Requirements include an Associate's degree in physical therapy, an active PTA license, and commitment to continuing education to maintain licensure. This full‑time role involves documentation in NetHealth EMR, treatment planning, resident and family education, and

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.
• Experience working in a skilled nursing facility is a plus.

Responsibilities

• Provide physical therapy services to residents under supervision.
• Conduct resident screenings and assist in evaluations as directed.
• Assist in developing and implementing individualized treatment plans.
• Deliver group and individual therapy sessions as outlined in the plan.
• Document care in NetHealth EMR in a timely, compliant manner.

Education

Associate's degree in physical therapy
