Job summary

Evolve Therapy Services, LLC is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility in Paris, TN. The PRN position supports therapy services under a supervising Physical Therapist, with responsibilities spanning evaluation, treatment planning, and resident education.

Requirements include an Associate's degree in Physical Therapy, an active PTA license, and commitment to ongoing professional development; NetHealth EMR experience is a plus.

Qualifications

• Associate's degree in Physical Therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.

Responsibilities

• Provide PT services to residents under supervision of a Physical Therapist.
• Conduct periodic screenings to determine intervention needs.
• Assist in developing goals and treatment plans under PT guidance.
• Prepare and implement treatment plans including mobility, strength, balance, and gait activities.
• Educate residents, families, and staff on therapy techniques and equipment use.
