Evolve Therapy Services is seeking a Physical Therapy Assistant to join an in-house skilled nursing facility in Torrington, CT. The role works under the supervision of a Physical Therapist to deliver resident-centered care and support therapy goals.

Responsibilities include screenings, treatment planning assistance, conducting individual and group sessions, documenting progress in NetHealth EMR, and educating residents, families, and staff.

Qualifikationen

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and professional development.
• Maintain licensure and certification.

Aufgaben

• Provide physical therapy services under the supervision of the Physical Therapist.
• Conduct screenings to determine need for intervention or treatment.
• Assist in evaluation and treatment planning with the Physical Therapist.
• Conduct individual and group treatment addressing strength, mobility, balance, gait and ROM.
• Document CMS-necessitated care in the NetHealth EMR system.
• Educate residents, families and staff on therapy services.
