Evolve Therapy Services in Torrington, CT is seeking a Physical Therapist for an in-house Skilled Nursing Facility. The role supports Full Time, Part Time, or PRN positions to provide therapy services and coordinate care for residents.

Responsibilities include developing treatment plans, documenting assessments, and collaborating with an interdisciplinary team to ensure quality resident care and CMS-compliant records.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and productivity.
• Document and bill PT services compliant with procedures.
• Evaluate and plan treatment and follow-up for resident programs.
• Screen residents to determine intervention needs and treatment timing.
• Develop treatment plans with short and long-term goals.
• Communicate findings and recommendations to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
