Job summary

Evolve Therapy Services is seeking a licensed Physical Therapy Assistant for an in-house Skilled Nursing Facility in Waterbury, CT. This role works under the supervision of a Physical Therapist to provide resident-focused therapy services and support the treatment plan.

The position offers Full Time, Part Time or PRN options with competitive pay and opportunities for growth. Responsibilities include screenings, interventions, documentation in NetHealth EMR, and coordination with the care team.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Identify indications for evaluations and assist in evaluating under the supervision of the Physical Therapist.
• Assist in establishing and modifying individual resident treatment plans with the PT.
• Deliver both individual and group therapy sessions focusing on strength, mobility, balance, gait, ROM, and modalities.
• Assist in selecting therapy techniques/media and sequence activities in coordination with the PT.
• Monitor residents' responses and report needed modifications to the PT.
• Help schedule residents' therapy sessions with the PT and care team.
• Adhere to department schedules and adjust as needed for treatment regimens.
• Educate residents, families, and staff on therapy techniques and equipment.
• Maintain accurate documentation in CMS and NetHealth EMR systems.
