Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant to join an in-house Skilled Nursing Facility team in Southport, CT. This part-time/PRN role requires providing PT services under supervision, conducting screenings, and helping develop and implement treatment plans for residents.

The position emphasizes collaboration with the Physical Therapist, proper documentation in NetHealth EMR, and resident/family education. Opportunities for growth exist within the therapy program.

Qualifications

• Active Physical Therapy Assistant license in applicable states.
• Associate's degree in physical therapy from an accredited program.
• Commitment to ongoing continuing education and licensure maintenance.

Responsabilités

• Provide physical therapy services to residents under the supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist in evaluations and development of treatment plans with the Physical Therapist.
• Conduct both individual and group treatment as outlined in treatment plans including strength, mobility, gait, ROM, balance, and modalities.
• Assist in selecting PT techniques/media and adapt as indicated by resident performance.
• Monitor residents' response to intervention and consult with PT for modifications.
• Help establish resident schedules with the PT and care team.
• Adhere to schedules and adjust for treatment regimens.
• Train residents, families, and staff on equipment use.
• Participate in resident, family, and staff education.
• Ensure timely, compliant documentation for CMS expectations in NetHealth EMR.
