Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Torrington, CT. The role supports residents with PT services, documentation, and interdisciplinary care.

Responsibilities include evaluations, goal setting, and communication with residents and families, with CMS-compliant NetHealth documentation. Positions available Full Time, Part Time, or PRN.

Qualifications

• Must hold a Bachelor of Science in Physical Therapy from an accredited program.
• Active PT license in state(s) of practice is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents per standards and productivity.
• Document and bill PT services according to procedures.
• Evaluate residents to guide treatment planning and implementation.
• Develop short- and long-term goals and treatment plans.
• Communicate findings and plans to residents, families, and interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
• Perform other duties to support the organization.

Education

Bachelor of Science Degree in Physical Therapy

Active Physical Therapy license

Continuing education to maintain licensure
