Job summary

Azria Health is seeking a Physical Therapy Assistant to provide direct rehab care under a Physical Therapist's supervision. The role requires licensure, a graduate degree in physical therapy, and CPR certification.

Prior post-acute care experience is preferred; proficiency with Word/Excel and therapy software is expected. Responsibilities include following treatment plans, documenting progress, and maintaining patient-focused care while collaborating with an interdisciplinary team.

Qualifications

• Must be a graduate of an accredited program in physical therapy (ASPT).
• Must be currently registered/licensed in applicable State and maintain active license.
• One year experience in post acute care or related setting preferred.
• CPR certification upon hire or during orientation; CPR must remain current.
• Proficient in Microsoft Word, Excel, and email.

Responsibilities

• Follow prescribed physical therapy treatment plans under the supervision of a licensed Physical Therapist.
• Chart patient progress accurately and timely.
• Utilize therapy software for documentation and charting.
• Provide excellent customer service and maintain a positive attitude toward patients.
• Assist in patient transfers and ensure safety and sanitation.
