You will provide screening, evaluation, and treatment, documenting progress and outcomes to support billing and care planning. A $5,000 sign-on bonus is offered to eligible candidates.

Qualifications

• Associates degree in Physical Therapy from an accredited program.
• SNF therapy experience preferred.
• Strong communication and compassionate approach.

Responsibilities

What You'll Do

• Provide individualized Therapy services including screening, evaluation, treatment, program planning, and implementation.
• Communicate patient progress, needs, and goals with residents, families, and the interdisciplinary care team.

• Support residents in regaining function, independence, and quality of life.

What You'll Need

• Associate Degree in Physical Therapy from an accredited Program.
