Job summary

Qualifications

• SNF therapy experience preferred.
• Strong communication and patient-centered approach required.

Responsibilities

• Provide individualized therapy services including screening, evaluation, treatment, program planning, and implementation.
• Communicate patient progress, needs, and goals with residents, families, and the care team.
• Complete documentation accurately and timely to support billing and payroll.

Position: Physical Therapy Assistant - Full Time.

What You'll Do:

• Provide individualized Therapy services including screening, evaluation, treatment, program planning, and implementation.
• Communicate patient progress, needs, and goals with residents, families, and the interdisciplinary care team.
• Complete documentation accurately and timely to support billing and payroll requirements.
• Support residents in regaining function, independence, and quality of life.

What You'll Need:

• Associate Degree in Physical Therapy from an accredited Program.
• Previous Skilled Nursing Facility (SNF) therapy experience
• Strong communication skills and a compassionate, patient-centered approach
