Freedom Pointe at The Villages in Florida is seeking a compassionate Physical Therapy Assistant to join our rehabilitation team. This full-time role focuses on personalized therapy, evaluation, treatment planning, and delivery of skilled services to residents in a senior living setting.

You will document progress, communicate goals with families and the care team, and support residents in regaining function and independence. A SNF therapy background and an associate degree are required.

Qualifications

• Associate Degree in Physical Therapy from an accredited program.
• Previous Skilled Nursing Facility (SNF) therapy experience.
• Strong communication skills and a compassionate, patient-centered approach.
• Background screening through AHCA -https://info.flclearinghouse.com

Responsibilities

• Provide individualized Therapy services including screening, evaluation, treatment, program planning, and implementation.
• Communicate patient progress, needs, and goals with residents, families, and the interdisciplinary care team.
• Complete documentation accurately and timely to support billing and payroll requirements.
• Support residents in regaining function, independence, and quality of life.
