Evolve Therapy Services, LLC is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Le Center, MN. This role supports the Physical Therapist by delivering therapeutic services and implementing treatment plans for residents in a facility setting.

The PTA will perform screenings, assist in evaluations, and help establish individual treatment goals. Applicants should hold an active PTA license in the state and pursue ongoing professional development.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention.
• Assist in evaluation under the direction of the Physical Therapist.
• Assist in forming treatment plans with short- and long-term goals.
• Conduct individual and group treatments focused on mobility, strength, balance, and ROM.
