Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in Le Center, MN. This part‑time or PRN role supports high‑quality resident care.

Qualifications

• Bachelor of Science Degree in Physical Therapy from an accredited program.
• Must have an active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Conduct evaluations and develop treatment plans with short and long‑term goals.
• Communicate evaluation findings and treatment plans to residents, families and the interdisciplinary team.
• Complete CMS‑compliant documentation in NetHealth EMR systems.
