Evolve Therapy Services in Worthington, MN is seeking a dedicated Physical Therapy Assistant to join an in-house skilled nursing facility team. You will work under the supervision of a Physical Therapist to deliver resident-centered therapy services.

Qualifications

• Associate's degree from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine intervention needs.
• Assist in evaluation and treatment planning under supervision.
• Help establish resident schedules with the Physical Therapist and team.
• Assist with and participate in resident education and staff training.
• Document compliant and medically necessary therapy activities in CMS-compliant EMR.
