Job summary

VitalCaring is seeking a Physical Therapy Assistant to deliver home health services, helping patients regain mobility and independence in their own homes. You will work one-on-one with patients, following individualized plans of care and using therapeutic modalities as appropriate.

Qualifications

• Graduate of an accredited Physical Therapy Assistant (PTA) program.
• Current PTA license or state-required certification.
• 1+ year of home health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct physical therapy services under licensed PT supervision.
• Follow individualized plans of care to improve mobility and function.
• Assist with therapeutic exercises, gait training, balance, and transfers.
• Document patient progress in the EMR accurately and timely.
• Collaborate with care teams to coordinate patient care.
