Job summary

Willow Valley Center for Nursing & Rehabilitation is seeking a compassionate Physical Therapist Assistant (PTA) for a flexible PRN position in a skilled nursing and long-term care setting. You will work with a licensed Physical Therapist and an interdisciplinary team to help residents regain mobility and independence.

The role focuses on resident-centered care with ongoing assessment, documentation, and collaboration.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current and valid PTA license in the state of practice.
• Prior experience in skilled nursing, rehabilitation, or long-term care preferred but not required.
• Ability to safely assist residents with transfers and mobility, including lifting up to 50 pounds.
• Strong communication, organization, and teamwork skills.

Responsibilities

• Provide skilled physical therapy treatments under the direction of a licensed Physical Therapist.
• Assist residents with mobility, balance, strength, and functional independence.
• Implement individualized treatment plans to meet resident goals.
• Document therapy sessions and progress per policies and regulations.
• Monitor resident response and communicate updates to the supervising PT.
• Assist with discharge planning and educate on home exercise programs.
• Collaborate with nursing and physician teams for coordinated care.
• Support department operations and coverage at affiliated facilities.
