Job summary

Evolve Therapy Services in Randallstown, MD is seeking a compassionate Physical Therapy Assistant for an in-house Skilled Nursing Facility. The role operates PRN under the supervision of a Physical Therapist.

You will conduct screenings, assist in evaluations, help develop treatment plans, and provide individual and group therapy focused on strength, mobility, balance, gait, and modalities. Documentation in NetHealth EMR is required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain current licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist the Physical Therapist in developing and implementing treatment plans and goals.
• Deliver individual and group treatments in strength, mobility, balance, gait, and modalities.
• Monitor residents' response to intervention and adjust plans as directed by the Physical Therapist.
• Maintain schedules with the Physical Therapist and other team members.
• Document compliant and medically necessary CMS-related records in NetHealth EMR.
