Job summary

Concordia Visiting Nurses seeks a compassionate Physical Therapy Assistant (PTA) to join our home health team. You will assist patients in regaining mobility under the supervision of a Physical Therapist, delivering exercises and gait training in the comfort of patients' homes.

The ideal candidate holds an accredited PTA program degree and a PA PTA license (or can obtain one before starting). CPR certification is required or obtainable prior to employment.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program and PA PTA license or ability to obtain.
• Proven ability to work under supervision and document patient progress.
• CPR certification or ability to obtain before employment.

Responsibilities

• Provide PTA services under the licensed Physical Therapist.
• Help patients regain strength, balance, mobility, and independence.
• Perform therapeutic exercises, gait and transfer training, and other interventions.
• Educate patients and caregivers on exercise programs and safety.
• Document progress and communicate changes to supervising PT.
• Collaborate with nurses and other health professionals to ensure quality care.

What you'll do as a Physical Therapy Assistant (PTA):

• Provide physical therapy treatments under the direction of a licensed Physical Therapist in accordance with the established plan of care.
• Assist patients in improving strength, balance, mobility, endurance, and functional independence.
• Implement therapeutic exercises, gait training, transfer training, and other evidence‑based interventions to support recovery.
• Educate patients and caregivers on exercise programs, fall prevention, safe mobility techniques, and the proper use of assistive devices.
• Observe, document, and report patient progress while communicating any changes to the supervising Physical Therapist.
• Collaborate with nurses, occupational therapists, speech therapists, physicians, and other healthcare professionals to provide exceptional patient care.
