Job summary

Concordia Lutheran Ministries is seeking a compassionate Physical Therapist to deliver high-quality, patient-centered care across Butler County, Armstrong County, Allegheny County, Westmoreland County, and Clarion County. You will conduct evaluations, develop plans, and support patients on their journey to recovery in a home health setting.

You will collaborate with physicians and a multidisciplinary team to apply manual therapy, exercises, and modalities while documenting progress and ensuring

Qualifications

• Graduate of an accredited Physical Therapy program.
• Licensed to practice in the state.
• Strong communication and interpersonal skills.
• Commitment to patient-centered care.

Responsibilities

• Conduct comprehensive evaluations and develop individualized treatment plans.
• Apply evidence-based therapeutic interventions and modalities.
• Monitor progress, adjust plans, and document thoroughly.

What You'll Do as Physical Therapist
