Job summary

Concordia Visiting Nurses is seeking a compassionate Physical Therapy Assistant to join our home health team. You will work with licensed Physical Therapists to help seniors regain strength, improve mobility, and remain independent in their homes.

You will provide one-on-one PT care, educate patients and families, and collaborate with an interdisciplinary team to ensure high-quality outcomes. PA PTA license and CPR certification are required or obtainable.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program with PA PTA license or ability to obtain prior to starting.
• Current CPR certification or ability to obtain prior to employment.

Responsibilities

• Provide physical therapy treatments under the direction of a licensed Physical Therapist in accordance with the established plan of care.
• Assist patients in improving strength, balance, mobility, endurance, and functional independence.
• Implement therapeutic exercises, gait training, transfer training, and other evidence-based interventions to support recovery.
• Educate patients and caregivers on exercise programs, fall prevention, safe mobility techniques, and the proper use of assistive devices.
• Observe, document, and report patient progress while communicating any changes to the supervising Physical Therapist.
• Collaborate with nurses, occupational therapists, speech therapists, physicians, and other healthcare professionals to provide exceptional patient care.
