Job summary

Walnut Creek Nursing and Rehab is seeking a dedicated Physical Therapy Assistant (PTA) to join our rehabilitation team in Erie, PA. New graduates are welcome to apply and opportunities for growth are available.

You will collaborate with Physical Therapists and the interdisciplinary team to improve residents’ strength, mobility, and independence, while documenting progress using Net Health. A compassionate, resident-centered approach is essential in our skilled nursing setting.

Qualifications

• Current or eligible Pennsylvania PTA license
• CPR certification preferred
• Strong communication and interpersonal skills
• Compassionate and patient-centered care
• Ability to safely assist residents with mobility and transfers
• Experienced PTAs and new graduates welcome

Responsibilities

• Implement individualized treatment plans established by the Physical Therapist
• Assist residents with functional mobility, transfers, ambulation, and assistive devices
• Guide residents through strengthening, balance, and therapeutic exercises
• Monitor resident progress and response to treatment
• Accurately document treatment and progress using Net Health
• Communicate effectively with therapists, nursing staff, residents, families and caregivers
• Provide compassionate, resident-centered care in a skilled nursing and long-term care setting

What You’ll Do

As a PTA, you’ll work closely with our Physical Therapists and interdisciplinary team to help residents improve their strength, mobility, independence, and overall quality of life.
