Job summary

Concordia Visiting Nurses is seeking a passionate Physical Therapist to join our close-knit team. You will evaluate patients, develop individualized treatment plans, and deliver evidence-based therapy in a home health setting, while educating families and coordinating with physicians to ensure comprehensive care.

What you'll do as a Physical Therapist PT:
