Job summary

CHI St. Joseph’s Health of Park Rapids, part of CommonSpirit Health, is seeking a Physical Therapist Assistant to help patients rebuild and maintain their physical wellbeing and improve their quality of life.

You will perform initial evaluations, create and update care plans, and deliver direct treatments in a collaborative, patient-centered environment. Ideal candidates are licensed in Minnesota with 1–3 years of experience, able to follow POC protocols, document services accurately, and work

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Licensed as a Physical Therapy Assistant in Minnesota.
• 1–3 years of work experience in physical therapy.

Responsibilities

• Perform initial evaluations and develop individualized plans of care.
• Provide a variety of direct treatments and adjust plans based on patient progress.
• Maintain timely and accurate documentation of services in the medical record.
• Collaborate with the interdisciplinary team and families to promote optimal outcomes.
• Meet patient care productivity standards and uphold quality care.

As our Physical Therapist, you will help patients rebuild or maintain their physical wellbeing so they can function at their best level, reduce pain, and improve their quality of life.

Every day you will perform initial evaluations, develop and update individualized plans of care, and provide a variety of direct treatments. You will be expected to consistently meet patient care productivity standards, collaborate with the interdisciplinary team and families, and promote a positive customer service environment.

To be successful in this role, you must possess strong age-appropriate competency skills, excellent communication abilities, and a collaborative, patient-centered approach to care.

• Demonstrates competence with clinical skills necessary to provide care appropriately to all the patient groups served by the physical therapy service
• Provides treatments as included in POC
• Provides instruction in/demonstration of equipment, treatments, disease process, self care and exercises to patients, family, and staff.
• Provides timely and accurate documentation of services provided in the medical record, per policy.
• Communicates with medical, nursing and other Rehab staff
• Staff will strive to provide service to patients or clients by filling our service promise of Kind, Compassionate Care, Every Person, Every Time.

Job Requirements

Required

• Graduate of a Physical Therapist Assistant program from an accredited institution
• Physical Therapy Assistant Licensed in the State of Minnesota
• 1-3 years of work experience

Pay Range

$24.94 - $37.09 /hour
