Transitions At Home for Southern Wisconsin is hiring a Full-Time Physical Therapist Assistant to help patients regain independence in their homes. This role involves administering physical therapy per physician orders under the guidance of the Administrator/Clinical Manager. The position offers a flexible schedule, supportive clinicians, and a focus on one-on-one patient relationships for compassionate care. Candidates must hold an Active Wisconsin Physical Therapist Assistant License and preferably have 2 years of experience.

Qualifications

• Must possess an Active Wisconsin Physical Therapist Assistant License.
• Having 2 years of relevant experience is preferred.
• CPR certification is required.
• Must hold a valid Wisconsin driver’s license and have car insurance.

Responsibilities

• Administer physical therapy as per physician orders.
• Assist patients in regaining independence at home.

Transitions At Home is hiring a Full‑Time Physical Therapist Assistant to help patients regain independence in their homes. The assistant administers physical therapy per physician orders and under the direction of the Administrator/Clinical Manager.

We offer a flexible schedule, independence, supportive clinicians, and one‑on‑one patient relationships for compassionate care.

Hours

• 8:00am – 4:30pm
• Monday through Friday
• No holidays
