Job summary

Qualifications

• Current unencumbered NYS Physical Therapy Assistant license.
• Graduate of an accredited program with an Associate’s degree in Physical Therapy.
• Geriatric experience preferred.
• A valid driver's license and reliable transportation.

Responsibilities

• Provides Physical Therapy plan of treatments and modalities under the supervision of a licensed PT.
• Communicates effectively with residents, families, and staff.
• Addresses residents with varying visual, cognitive, auditory, and physical needs.
• Trains, supervises and disciplines subordinate staff as needed.
• Assists in planning, organizing and implementing department goals and policies.
• Documents resident progress toward PT goals.
• Completes daily activity schedules and progress notes per policy.
• Provides education to residents, staff, and families as directed.
• Complies with federal and state rehab regulations and documentation standards.
• Assists in ordering equipment and maintains facility policies and confidentiality.
• Keeps Rehabilitation manager and PT informed of updates.

Qualifications/Experience:

• Must possess a current unencumbered NYS license as an Physical Therapy Assistant

• Graduate of an accredited school with an Associate’s degree in Physical Therapy

• A current unrestricted driver’s license and dependable transportation

Essential Job Functions:

• Provides Physical Therapy plan of treatments and modalities according to the established treatment plans as assigned, under the on-site supervision of a licensed Physical Therapist

• Must possess the ability to deal tactfully and effectively communicate with personnel, residents, family members, visitors, and the general public

• Must effectively communicate with residents with visual, speech, auditory, cognitive, physical and emotional impairments

• Must possess the ability to train/supervise and discipline subordinate staff

• Must possess the ability to assist in planning, organizing and implementing the goals, objectives and policies and procedures of the department

• Documents resident progress toward Physical Therapy goals

• Completes Daily Activity Schedule and progress notes per facility policy and as directed by the Physical Therapist

• Provides resident, staff and family education as directed

• Complies with Federal and State regulations related to rehabilitation therapy

• Completes all documentation to comply with professional standard and regulatory requirements

• Assists in ordering necessary equipment for the facility and residents

• Complies with all facility policies and procedures

• Maintains confidentiality of facility and resident information

• Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately

• Keeps Facility Rehabilitation manager and Physical Therapist informed and updated appropriately
