Job summary

Personal Healthcare Management LLC is seeking a licensed Physical Therapy Assistant in New York to deliver PT plans under supervision, and to support residents' mobility and rehabilitation goals.

The position requires an Associate’s degree in Physical Therapy and a NYS license, with geriatric experience preferred. A valid driver’s license and reliable transportation are expected. The role emphasizes documentation, safety, and regulatory compliance.

Qualifications

• Must possess NYS license as a Physical Therapy Assistant.
• Graduate of an accredited school with an Associate’s degree in Physical Therapy.
• Geriatric experience preferred.
• A current unrestricted driver’s license and dependable transportation.

Responsibilities

• Provides Physical Therapy plan of treatments and modalities according to the established treatment plans as assigned, under the on-site supervision of a licensed Physical Therapist
• Must possess the ability to deal tactfully and effectively communicate with personnel, residents, family members, visitors, and the general public
• Must effectively communicate with residents with visual, speech, auditory, cognitive, physical and emotional impairments
• Must possess the ability to train/supervise and discipline subordinate staff
• Must possess the ability to assist in planning, organizing and implementing the goals, objectives and policies and procedures of the department
• Documents resident progress toward Physical Therapy goals
• Completes Daily Activity Schedule and progress notes per facility policy and as directed by the Physical Therapist
• Provides resident, staff and family education as directed
• Complies with Federal and State regulations related to rehabilitation therapy
• Completes all documentation to comply with professional standard and regulatory requirements
• Assists in ordering necessary equipment for the facility and residents
• Complies with all facility policies and procedures
• Maintains confidentiality of facility and resident information
• Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately
• Keeps Facility Rehabilitation manager and Physical Therapist informed and updated appropriately
