Job summary

Wells-Nursing-Home, Inc. seeks a licensed Physical Therapy Assistant to provide PT services under the supervision of a licensed Physical Therapist.

The role emphasizes independent work within a team, geriatric-focused care, and strong communication with residents and families. Must have NYS PTA license and an associate degree in physical therapy; a valid driver's license and reliable transportation are required.

Qualifications

• Must possess a current unencumbered NYS license as a Physical Therapy Assistant.
• Graduate of an accredited school with an associate's degree in physical therapy.
• Geriatric experience preferred.
• A current unrestricted driver’s license and dependable transportation.
• Ability to function independently as well as part of a team.

Responsibilities

• Provides Physical Therapy plan of treatments and modalities according to the established treatment plans under the on-site supervision of a licensed Physical Therapist.
• Communicate tactfully with personnel, residents, family members, visitors, and the general public.
• Communicate with residents with various impairments (visual, speech, auditory, cognitive, physical, emotional).
• Train/supervise and discipline subordinate staff as needed.
• Relate information regarding a resident’s condition to the care team.
• Assist in planning, organizing and implementing department goals, objectives and procedures.
• Document resident progress toward Physical Therapy goals.
• Complete daily activity schedules and progress notes per policy and directed by the PT.
• Provide resident, staff and family education as directed.
• Comply with federal and state regulations related to rehabilitation therapy.
• Complete all documentation to comply with professional standards and regulatory requirements.
• Assist in ordering necessary equipment for the facility and residents.
• Maintain confidentiality of facility and resident information.
• Report any instances of potential resident abuse, neglect, or misappropriation immediately.
• Follow sanitation and infection control policies and procedures.
• Keep Rehab Manager and PT informed and updated appropriately.
