Job summary

Personal Healthcare Management LLC seeks an experienced Physical Therapy Assistant to join our on-site team. The role supports treatment plans under the supervision of a licensed Physical Therapist, delivering modalities and documenting progress in resident care settings.

Qualifications

• Current NYS license as a Physical Therapy Assistant (unencumbered).
• Associate degree in physical therapy from an accredited program.
• Geriatric care experience preferred.
• Valid driver license and reliable transportation.
• Ability to work independently and as part of a team.

Responsibilities

• Provide PT plans and modalities under supervision of a licensed Physical Therapist.
• Communicate effectively with residents, families, visitors and staff.
• Interact with residents with varying impairments (visual, cognitive, etc.).
• Train, supervise and discipline subordinate staff as needed.
• Relate information about residents' conditions to the team.
• Assist in planning and implementing department goals and procedures.
• Document progress toward Physical Therapy goals.
• Complete daily activity schedules and progress notes per policy.
• Educate residents, staff and families as directed.
• Comply with federal and state rehabilitation regulations.
• Complete documentation to meet professional and regulatory standards.
• Assist in ordering equipment for facility and residents.
• Maintain confidentiality of facility and resident information.
• Report suspected resident abuse or neglect immediately.
• Follow sanitation and infection control policies.
• Keep Rehabilitation manager and Physical Therapist informed.

Qualifications/Experience:

• Must possess a current unencumbered NYS license as an Physical Therapy Assistant

• Graduate of an accredited school with an associate's degree in physical therapy

• Geriatric experience preferred

• A current unrestricted driver’s license and dependable transportation

• Ability to function independently as well as part of a team

Essential Job Functions:

• Provides Physical Therapy plan of treatments and modalities according to the established treatment plans as assigned, under the on-site supervision of a licensed Physical Therapist

• Must possess the ability to deal tactfully and effectively communicate with personnel, residents, family members, visitors, and the general public

• Must effectively communicate with residents with visual, speech, auditory, cognitive, physical and emotional impairments

• Must possess the ability to train/supervise and discipline subordinate staff

• Must be able to relate information regarding a resident’s condition

• Must possess the ability to assist in planning, organizing and implementing the goals, objectives and policies and procedures of the department

• Documents resident progress toward Physical Therapy goals

• Completes Daily Activity Schedule and progress notes per facility policy and as directed by the Physical Therapist

• Provides resident, staff and family education as directed

• Complies with Federal and State regulations related to rehabilitation therapy

• Completes all documentation to comply with professional standard and regulatory requirements

• Assists in ordering necessary equipment for the facility and residents

• Complies with all facility policies and procedures

• Maintains confidentiality of facility and resident information

• Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately

• Follows sanitation and infection control policies and procedures

• Keeps Facility Rehabilitation manager and Physical Therapist informed and updated appropriately
