Job summary

Personal Healthcare Management LLC is hiring a Physical Therapy Assistant in New York. The role delivers PT treatments under supervision, supports residents and families, and collaborates with a care team to meet rehab goals in a facility setting.

Candidates must hold a NYS PTA license, have an associate degree, and demonstrate the ability to work independently and as part of a team. Geriatric experience is preferred, with a valid driver's license and reliable transportation.

Qualifications

• Must possess a current unencumbered NYS license as an Physical Therapy Assistant
• Graduate of an accredited school with an Associate’s degree in Physical Therapy
• Geriatric experience preferred
• A current unrestricted driver’s license and dependable transportation
• Ability to function independently as well as part of a team

Responsibilities

• Provides Physical Therapy plan of treatments and modalities under the supervision of a licensed PT.
• Communicate tactfully with personnel, residents, families, visitors and the public.
• Communicate with residents with visual, speech, auditory, cognitive, physical and emotional impairments.
• Train/supervise and discipline subordinate staff as needed.
• Relay information regarding a resident’s condition to the team.
• Assist in planning, organizing and implementing department goals and procedures.
• Document resident progress toward Physical Therapy goals.
• Complete Daily Activity Schedule and progress notes per policy and as directed by the PT.
• Provide resident, staff and family education as directed.
• Comply with Federal and State regulations related to rehabilitation therapy.
• Complete all documentation to meet professional standards and regulatory requirements.
• Assist in ordering necessary equipment for the facility and residents.
• Comply with all facility policies and procedures.
• Maintain confidentiality of facility and resident information.
• Report all instances of potential resident abuse, neglect, and misappropriation immediately.
• Follow sanitation and infection control policies and procedures.
• Keep Rehabilitation manager and Physical Therapist informed and updated.
