Job summary

Centerville Post Acute in Centerville, OH is seeking a licensed Physical Therapist Assistant (PTA) to join our rehabilitation team under the supervision of a licensed Physical Therapist. The PTA will help residents restore mobility, improve strength, and achieve their highest functional independence.

Responsibilities include administering treatments, assisting with transfers and gait training, documenting progress in EMR, educating residents and families, and collaborating with nurses, OTs,

Qualifications

  • Graduate of an accredited PTA program.
  • Current Ohio PTA license.
  • CPR certification required.
  • Minimum 2 years PTA experience preferred, with SNF/post-acute care preferred.
  • Excellent communication and interpersonal skills.

  • Administer physical therapy treatments and therapeutic exercises as directed by the supervising Physical Therapist.
  • Assist residents in improving mobility, strength, balance, coordination, endurance, and functional independence.
  • Monitor resident progress and communicate changes to the supervising Physical Therapist.
  • Accurately document treatments, resident responses, and progress in the electronic medical record.
  • Educate residents and family members on exercise programs, safety precautions, and proper use of assistive devices.
  • Assist with transfers, gait training, therapeutic activities, and functional mobility exercises.
  • Participate in interdisciplinary care planning meetings.
  • Maintain treatment areas, equipment, and supplies in a clean and safe condition.
  • Follow all facility policies, state and federal regulations, and Medicare documentation requirements.
  • Collaborate with nursing staff, occupational therapists, speech therapists, physicians, and other members of the healthcare team.

Job Type: Full-Time

Position Summary

Centerville Post Acute is seeking a compassionate, motivated, and licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. Working under the direction and supervision of a licensed Physical Therapist, the PTA plays a key role in helping residents restore mobility, improve strength, reduce pain, and achieve their highest level of functional independence.

Essential Responsibilities

  • Ability to safely lift, transfer, reposition, and assist residents using appropriate equipment and safe patient handling techniques.
  • Ability to occasionally lift up to 50 pounds.
