Centerville Post Acute in Centerville, OH is seeking a licensed Physical Therapist Assistant (PTA) to join our rehabilitation team. Working under a Physical Therapist, you will help residents regain mobility, build strength, reduce pain, and achieve independence in daily activities.

The ideal candidate has an accredited PTA program graduate, an Ohio PTA license, CPR certification, and at least 2 years of PTA experience, preferably in skilled nursing or post-acute care.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Ohio PTA license.
• CPR certification required.
• Minimum 2 years PTA experience; SNF/post-acute care preferred.

Responsibilities

• Administer PT treatments and therapeutic exercises as directed by the supervising Physical Therapist.
• Assist residents in improving mobility, strength, balance, coordination, endurance, and functional independence.
• Monitor resident progress and communicate changes to the supervising Physical Therapist.
• Accurately document treatments, resident responses, and progress in the electronic medical record.
• Educate residents and family members on exercise programs, safety precautions, and proper use of assistive devices.
• Assist with transfers, gait training, therapeutic activities, and functional mobility exercises.
• Participate in interdisciplinary care planning meetings.
• Maintain treatment areas, equipment, and supplies in a clean and safe condition.
