Job summary

Centerville Post Acute in Centerville, OH is seeking a licensed Physical Therapist to join our rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide skilled therapy to maximize independence and mobility in a skilled nursing setting.

The role emphasizes collaboration with physicians and interdisciplinary staff, documentation accuracy, and staying current with evidence-based practices.

Qualifications

  • Graduate of an accredited Physical Therapy program.
  • Current Physical Therapist license in the State of Ohio.
  • CPR certification required.
  • Minimum of 2 years of physical therapy experience preferred, with SNF or post-acute care experience strongly preferred.
  • Excellent communication, organization, and clinical decision-making skills.

Responsibilities

  • Evaluate residents and develop individualized physical therapy treatment plans.
  • Provide skilled therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence.
  • Establish measurable treatment goals and modify care plans based on resident progress.
  • Educate residents, families, and caregivers on exercises, safety techniques, and discharge recommendations.
  • Complete timely and accurate clinical documentation in accordance with facility, state, federal, and payer requirements.
  • Participate in interdisciplinary care planning meetings.
  • Collaborate with physicians, nurses, OT, speech therapists, and other healthcare professionals.
  • Recommend adaptive equipment, assistive devices, and environmental modifications as appropriate.
  • Ensure compliance with Medicare, Medicaid, and rehabilitation documentation guidelines.
  • Maintain current knowledge of evidence-based physical therapy practices and regulatory standards.

Centerville Post Acute is seeking a compassionate, licensed, and dedicated Physical Therapist (PT) to join our rehabilitation team. Our therapists play a vital role in helping residents regain strength, mobility, independence, and quality of life following illness, injury, or surgery. We are looking for a team-oriented professional who is passionate about providing exceptional patient-centered care in a skilled nursing and rehabilitation setting.

Position Summary

The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing skilled physical therapy services that maximize functional independence and improve overall quality of life. The Physical Therapist collaborates with physicians, nursing staff, and the interdisciplinary team to ensure the highest level of resident care.

Essential Responsibilities
