Job summary

Trilogy Health Services, LLC is seeking a Physical Therapist to provide comprehensive PT services, including assessment, treatment planning and therapeutic interventions in a collaborative setting. You will work within an interdisciplinary team to optimize patient function and outcomes.

Qualifications

• Must hold a current, valid state PT licensure or certification.
• Education: Degree in Physical Therapy from an accredited program.
• Experience: 0-1 years in physical therapy setting.

Responsibilities

• Conduct thorough assessments to evaluate patients’ physical conditions and rehabilitation needs.
• Develop and implement personalized treatment plans with exercises, manual therapy and modalities.
• Educate patients and families on exercise techniques, posture and body mechanics.
• Maintain accurate documentation of patient progress, plans and outcomes.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.
